Varenicline is the most effective single medication for smoking cessation, and the most common reason a quit attempt fails on it is running out mid-course. A twelve-week course interrupted at week six is not a shorter course — it is usually a relapse.

Why continuity decides the outcome

The standard course runs twelve weeks, with a further twelve weeks for patients who have quit and want to consolidate. The evidence for varenicline's superiority over nicotine replacement and bupropion rests on completed courses. Dropping out early is the single strongest predictor of returning to smoking.

Prescription gaps cause a lot of that dropout. A prescriber who becomes unavailable in week five, an insurance change, a pharmacy that cannot reach anyone for authorisation — and a quit attempt that was working stops. That gap is what this service is for.

The standard titration, and where you should be

Varenicline starts with a one-week ramp: 0.5 mg once daily for three days, 0.5 mg twice daily for four days, then 1 mg twice daily as the maintenance dose. Some patients stay at 0.5 mg twice daily because of nausea, and that is a legitimate maintenance dose.

We continue the dose you are established on. We do not run the initial titration — a new start needs a prescriber who can set a quit date with you and follow the first fortnight, which is when most of the adverse effects and most of the dropout happen.

What the physician reviews

Nausea is the commonest adverse effect, affecting a large minority of patients, and it is usually manageable by taking doses with food and a full glass of water. Vivid or abnormal dreams and insomnia are also frequent and are worth distinguishing from anything more concerning.

We screen specifically for new or worsening mood symptoms — depression, agitation, or any thoughts of self-harm. The prominent neuropsychiatric boxed warning was removed after later trial evidence, but that is not the same as the question being irrelevant for an individual patient, particularly someone with a psychiatric history. Nicotine withdrawal itself affects mood, which is exactly why it needs a physician's judgement rather than a checkbox.

Also reviewed: significant renal impairment, which requires dose reduction; a seizure history; and cardiovascular disease, which warrants attention even though the evidence has been reassuring.
